Jami Falk is an Office of Primary Care (OPC) Nurse Leader with over 21 years of progressive leadership experience within the Veterans Health Administration, spanning critical care, mental health, ambulatory care, and enterprise-level quality management, and primary care. She has extensive expertise in advancing primary care nursing practice, population health, and system-wide quality improvement across facility, regional, and national levels.

For the past 15 years in ambulatory care, Ms. Falk has led the development of primary care nursing practice guidance and transformational coaching initiatives focused on population health, chronic disease management, and post-discharge quality improvement. She is committed to improving care for underserved populations, expanding access to women’s health services, and advancing rural health through telehealth.

Ms. Falk provides executive oversight to ensure safe, efficient, and high-quality nursing practice, leading process improvement efforts through effective change management strategies.
